Changing Long Term Care

When entering into a nursing facility many have a difficult transition. Many struggle to find meaning and purpose in this stage of life and often residents and families experience grief and loss. We want to help create a community in which individuals in this setting have access to quality behavioral health care

All our providers are mobile and travel to the facilities to provide in house services. No need to hassle with transportation and long wait times to be seen. We come to you! We also provide telehealth options for urgent and same day appointments

Not everyone has the same needs and we offer a variety of different services to fit the different needs of those we serve. We have an individualized approach and want to change the cookie cutter approach to mental health care

We believe everyone has a life story and we believe in individuals ability to heal and grow. Our providers have many years of experience in this field and want every patient to be met with empathy, dignity and respect

Insurance LIST

Medicaid, Medicare, Optum, United Healthcare, Regence BCBS, Aetna, Select Health

The above list of insurance are those which we are in network with. We are working on obtaining other insurance contracts. Please reach out to find out if we are in network. 

Private Pay

If we are not in network we offer a discounted cash pay for services. Please reach out for a cost of self pay.

We currently offer services in Utah County, Salt Lake County, Davis County and Weber County

Our non profit was created in order to serve and reach more patients. All funds received are used to broaden the scope of services we can provide to individuals residing in long term care and improve their quality of life.
